Terms and Conditions

SilverSoul Medical Supplies LLC

These terms explain how to use the Contact Us form at silversoulcare.com for general inquiries, complaints, exchange requests and information opt-outs. They apply to the contact form and our response to your request. Please also read our Website Privacy Policy. Submitting a message does not place an order or authorize a charge.

Purpose of the contact form

Use this form to ask about SilverSoul services, raise a concern, request help with an item or communicate your privacy and contact preferences. SilverSoul supplies off-the-shelf orthopedic braces within its authorized scope. Availability depends on the item, location and applicable requirements. A website listing or inquiry response does not establish insurance coverage, supplier enrollment, network participation or approval of an order.

Emergencies and medical advice

The form and email inbox are not emergency services and are not monitored continuously. For a medical emergency in the United States, call 911. Do not wait for a website response. Contact your treating clinician for diagnosis, treatment decisions or advice about symptoms. General information from SilverSoul does not replace professional medical advice or establish a clinician-patient relationship.

Information you submit

Provide accurate contact details and a brief description of your request. Use an email address you control and tell us if a different response method is needed. Do not impersonate another person or submit information you are not entitled to share. If contacting us for someone else, state your relationship; we may need to verify identity and authority before disclosing records or acting on their behalf. A family relationship alone does not establish authority.
The form is intended for adults. A parent, guardian or appropriately authorized representative should contact us when assistance for a minor is needed. Children under 13 should not submit information through this general form.

Sensitive information and privacy

Do not enter Social Security numbers, Medicare numbers, insurance identification numbers, payment-card details, passwords or detailed medical records in the general message field. If additional information is needed, we will arrange an appropriate collection method. Our Website Privacy Policy explains information handling, including authorized support services outside the United States. Patient health-information rights are addressed separately in our Notice of Privacy Practices.
Accepting these terms or acknowledging the privacy policy does not replace a HIPAA authorization or waive privacy rights. Ordinary email may carry privacy risks; requesting an email reply is not blanket permission to send sensitive records by ordinary email. These terms do not remove our responsibility to protect information under applicable law.

Responses and contact permission

Submitting the form asks SilverSoul to review your message and respond about that request, ordinarily using the email address provided. Our team responds during office hours. Routine response times vary, and an automated acknowledgment confirms receipt only. If the form fails or you need assistance, use the contact details below.
Submitting this form does not subscribe you to unrelated marketing or authorize automated calls, prerecorded calls or texts. We will obtain any required permission before initiating telephone follow-up. A phone number in a message, a privacy acknowledgment or acceptance of these terms alone is not supplier-specific telephone permission. You may request a different communication method or ask us to stop optional communications.

Orders insurance and financial responsibility

There is no SilverSoul charge for submitting an inquiry, reviewing eligibility or requesting your primary care doctor’s approval. Under SilverSoul policy, we must receive and verify your primary care doctor’s valid written order before supplying the requested item, submitting an insurance claim or billing or collecting payment for that order. If approval is declined or not received, SilverSoul does not supply the item and bills neither you nor your insurer for that order.
Doctor approval does not guarantee insurance payment or by itself create a payment obligation. You must also choose to proceed after receiving information about the item, expected costs and any required notices. Applicable delivery, documentation, payer and legal requirements must be met. Any existing, lawfully incurred obligation for a separate completed order remains separate from your contact-form submission.
Our Financial Obligations Information explains possible patient costs and billing protections. These contact terms do not authorize charges, recurring payments, assignment of benefits or responsibility for every insurance denial. They do not replace a required Advance Beneficiary Notice of Noncoverage or other payer notice, and do not waive Qualified Medicare Beneficiary protections or other billing and appeal rights.

Complaints and service concerns

You may complain through this form, by telephone, by email or in writing. Select Complaint and describe the issue briefly; we will arrange secure collection of any sensitive details needed. We acknowledge complaints within five calendar days of receipt and provide written investigation results within fourteen calendar days. Office closures do not restart these periods.
You will not be penalized for raising a concern or exercising your rights. You may request management review and contact your insurer, Medicare or an appropriate regulator without first completing our internal process. Contacting SilverSoul does not extend a legal, insurer or appeal deadline. A complaint or privacy request does not require agreement to new financial or marketing terms.

Exchange return and product requests

An Exchange Request asks us to review the issue; submission alone does not approve an exchange, refund or replacement. Briefly identify the item and problem without including unnecessary medical information. Contact us for return instructions before mailing an item. The applicable product terms, warranty, condition of the item and legal requirements govern the resolution. We will explain any proposed charge before you agree to it.
These terms do not remove required return rights, including the applicable Medicare supplier obligation to accept returns of substandard or unsuitable items. We will review affected billing and correct or refund amounts when required. Do not dispose of a disputed or recalled item before obtaining safe handling instructions unless immediate safety needs require otherwise.

Information opt-outs and communication preferences

Select Information Opt Out to request that optional communications stop or to ask about your information. You may also contact us directly. For an email marketing opt-out, only the email address to suppress is needed; no fee, explanation, account or acceptance of new terms is required. We honor marketing email opt-outs within ten business days and other requests within applicable deadlines. We do not use an opt-out request to enroll you in marketing.
Stopping optional messages does not automatically cancel an existing order, erase records that must be retained or prevent notices that are required or otherwise lawfully necessary for an existing service matter. We will respect applicable contact restrictions and reasonable communication preferences. Tell us separately if you also want to cancel an order or exercise a privacy right.

Privacy rights requests

You may ask for access, correction, deletion or other privacy assistance by contacting us and asking for the Privacy Officer. We may verify identity and authority where appropriate to protect records. Such verification is distinct from a simple email marketing opt-out. We will explain applicable limits, respond within the required period and provide information about any available review or appeal process. A deletion request does not require destruction of records that law requires us to keep.

Appropriate use and website availability

Do not use the form for spam, threats, impersonation, malicious software or attempts to access another person’s information. We may restrict abusive or technically harmful use while preserving reasonable alternative ways to make legitimate complaints and exercise rights. An unfavorable review, disagreement or good-faith complaint is not misuse.
Website information may require clarification, and technical interruptions can occur. If a submission fails, contact us directly. External maps and links are provided for convenience and operate under their providers’ terms. This does not exclude responsibility that applicable law places on SilverSoul or limit a remedy that cannot lawfully be excluded.

Your rights and other documents

Applicable federal and state law governs these terms, including mandatory protections that apply where you live. Our Website Privacy Policy explains website data handling; our Notice of Privacy Practices addresses patient health-information practices; our Financial Obligations Information addresses possible order costs. Order-specific documents apply only through the appropriate separate process. No provision removes rights, warranties, complaint options or billing protections that cannot lawfully be waived. If a provision is unenforceable, the remaining provisions apply to the extent permitted by law.

Changes to these terms

We may update these terms from time to time. Changes apply prospectively after the revised terms are posted. We will provide any additional notice or choice required by law. Changes do not retroactively create payment obligations, authorize new marketing or remove rights concerning an earlier submission.
